Recertification Application: How to Print Application

 If you are looking for a paper application to complete the recertification process, you can use information below.


There are a couple of ways to obtain a paper application. Please note, submitting an electronic application is the quicker option to completing your renewal.*
 

Option 1: Print Completed Application: 

You can use your National Registry account to enter all of the education electronically, before printing a copy.
  1. From your National Registry account, go to My Certification Dashboard 
  2. Click View Account Records 
  3. Locate Print Recertification Application (applications are listed by Recertification Cycle, select the current cycle)
  4. Click Print This Application 

Option 2: Blank Recertification Application:

Use the links below to print the necessary level

*Please be aware mailed application will take between 6-8 weeks for processing and should be sent in a traceable manner.
